By accessing or using this website operated by Four Seasons Therapy & Wellness (“the Practice”), you agree to be bound by these Terms of Use and our Privacy Policy. If you do not agree to these terms, do not use this site or submit any information through it.

Purpose of This Site

This website is used exclusively to collect intake information from prospective and current clients of the Practice. It is not a medical portal, emergency service, or substitute for in-person clinical care. If you are experiencing a mental health crisis, please call 988 (Suicide & Crisis Lifeline), go to your nearest emergency room, or call 911.

Consent to Data Collection & Processing

By submitting an intake form on this site, you consent to the collection, storage, and processing of the information you provide, as described in our Privacy Policy. This includes the use of AI-assisted tools to help your practitioner prepare for your sessions. All such processing is performed under HIPAA-compliant safeguards, including hardware-isolated execution environments and zero data retention, as detailed in the Privacy Policy.

Accuracy of Information

You agree to provide truthful and accurate information when completing intake forms. The Practice relies on the information you submit to prepare for your clinical care. Providing false or misleading information may affect the quality of care you receive.

No Practitioner–Client Relationship from Site Use

Submitting an intake form does not establish a practitioner–client relationship. A therapeutic relationship is established only when the practitioner has reviewed your information, determined that services are appropriate, and you have both agreed to proceed with treatment.

SMS & Text Message Communications

By providing your phone number and using this site, you consent to receive automated SMS text messages from Four Seasons Therapy & Wellness for the purpose of identity verification (one-time passcodes). These messages are necessary to securely access your intake forms and submitted information.

  • Message frequency: You will receive one text message each time a verification code is requested. No marketing or promotional messages will be sent.
  • One-way messages: Verification codes are sent via one-way SMS. Replies to these messages are not monitored or supported.
  • Message and data rates may apply: Standard messaging and data rates from your wireless carrier may apply to messages you receive.
  • Carriers supported: Service is available on most major U.S. carriers. The Practice is not responsible for delayed or undelivered messages due to carrier issues.
